How to apply for a state-funded assistive device

In order to receive state-funded technical aids, certain documents must be prepared and submitted. This section contains information on what is required in each situation to make the application process as easy as possible for you.

Required documents when applying for the first time

If you are applying for an assistive device (AD) for the first time, you need to start by preparing two main basic documents: 

  1. Medical professional's opinion in the e-health system 
  2. Your application.

Application for Urgent Technical Aids

If technical aids are needed urgently (injury, surgery, work), in addition to the basic documents, a document proving your rights must be submitted.
If you need technical aids urgently, the application must include a document that confirms your right to priority treatment.
Accordion: Documents to Justify Urgency

  1. Persons with first-time functional impairments: A medical record extract or discharge summary not older than 6 months must be submitted.
  2. Persons with predictable disabilities: An individual rehabilitation plan must be submitted, in which the specialist clearly states that technical aids are needed urgently.
  3. Employed persons: An employer's confirmation (not older than 1 month) must be submitted, stating that technical aids are necessary for performing direct work duties.
  4. Students: A confirmation from the educational institution regarding the need for technical aids in the learning process must be submitted.
  5. Self-employed persons or business operators: The company name and taxpayer registration number must be provided (applicable if technical aids are needed for work).
  6. Pregnant women: If technical aids are needed due to changes in body anthropometric parameters (height).
  7. Children and youth: Children, as well as those who have reached adulthood, if they entered the queue at least 6 months before reaching adulthood.
