Prepare the documents

Both initial and repeated applications require the same basic documents.

- you need to contact a family doctor, treating doctor, or specialist
- the opinion is prepared in the E-health system
  Application
- can fill out the form link to the application form;
- can be prepared together with a specialist
- can be written in free form, mandatory to indicate:
  1. name, surname
  2. personal code
  3. address
  4. phone, email
  5. required assistive device
  Representation documents
- if documents are submitted by another person, a power of attorney must be attached
