Ways to Submit Documents

To apply for receiving assistive devices, you need to submit an application and the necessary documents. This can be done in the way that is most convenient for you - electronically, in paper form, or in person.

The application can be signed:

  • with a secure electronic signature
  • or in paper form (by hand)

After signing, documents can be submitted in several ways.

  • Documents must be signed with a secure e-signature

Submitting Documents in the Client Portal

  • Log in to the client portal using an e-signature
  • Fill out the application form online
  • Attach the necessary documents
  • Confirm the application with an e-signature or another secure method

The client portal is a convenient and secure place to apply for technical aids and submit documents. It is available at any time and from any device.

Here you can:
   • submit applications and attach documents,
   • schedule appointments or receive/return assistive devices,
   • perform service operations online.
