Required documents for receiving an assistive device

To apply for receiving an assistive device, certain documents must be submitted. Below you will find information on what is needed and who prepares these documents.

1. Opinion for receiving an assistive device

First, you need to go to:

  • a family doctor,
  • or a treating doctor,
  • or a certified functional specialist (occupational therapist, physiotherapist, technical orthopedist, or audiologist).

The specialist will prepare an opinion for receiving the assistive device in the E-health system.

2. Application for receiving TPL 

To request an assistive device, an application must be submitted.

How to prepare the application

You can:

The application must include:

  • name, surname, and personal code,
  • residential address,
  • phone number,
  • email address or official e-address,
  • type of assistive device needed and purpose of use (for example, mobility indoors or outdoors).

If the application is signed by an authorized person, a copy of the authorization must be attached.

3. Additional documents for certain assistive devices

Some technical aids require additional documents that confirm the specific need.

Documents for urgent cases

In some situations, an assistive device is needed especially urgently, for example:

  • after an injury or surgery,
  • in case of deterioration of health,
  • due to educational or work circumstances.

In such cases, an additional document must be attached to the application, which confirms the right to receive an assistive device urgently.

Who can apply urgently?

  • persons with primary functional disorders,
  • persons with predictable disability,
  • due to educational or work circumstances.
  • children (including those who have reached adulthood, if they applied at least 6 months before reaching adulthood),
  • pregnant women, if the assistive device needs to be changed due to bodily changes,
  • persons who are studying and need an assistive device for learning,
  • working persons who need an assistive device for work.

What documents must be submitted in urgent cases

  • Primary disorders - extract from the medical record or epikrisis (not older than 6 months)
  • Predictable disability - individual rehabilitation plan indicating urgency
  • Learning - certificate from the educational institution (does not apply to assistive devices for care, protection, household, and space adaptation)
  • Employment - employer's certificate (not older than 1 month)
  • Self-employment - taxpayer number and company name (does not apply to assistive devices for care, protection, household, and space adaptation)

    The original application signed by a person can be submitted: 
    in person at any VTPC department (link to department contacts), 
    by sending electronically to the email vtpc@nrc.lv or  NRC “Vaivari” official electronic address (e-address), (must be signed with a secure electronic signature (e-signature).

  • Family members can sign the application for the allocation of technical aids if there is a notarized power of attorney, attaching a copy of the power of attorney.

  • For all services of the Vaivari Technical Aids Centre, it is possible to pay both with a bank card and in cash. This is also possible for services during home visits and outings.
